During Weekly contacts with clients's including periodic face-to-face visit we will assess the following:
- How to help the client identify their needs.
- How to help client deal with emergencies.
- Provide the information and assistance needed for the client to gain access to services.
- Give emotional support to the client as they deal with the effects of their illness.
- Help client's obtain the skills they need to manage resources the access services.
- Educate clients about HIV and treatment.
- Negotiate with service providers and advocate for client to help them access services.
The central goal for Unlimited Alternatives Case Management is to help our consumers function at the highest level possible and increase clients understanding of HIV transition and
treatment. Unlimited Alternatives Case management strive to assist clients in ways that are culturally sensitive, compassionate, non-judgmental, and that projects clients
privacy.
